Note: The original Record Book was owned by Alva Mae (Swan) Hooker in Temple, Texas.  Glenn Blair had a copy made of the book on July 16, 1981 and gave it to his son, Billy Blair, who began making  this transcription in November, 2012.  The small holes on the sides of the pages were made from the binding of the copy, not from the original record book.
